I used to play Call of Duty: Modern Warfare pretty dedicatedly so I ended up achieving a few prestige ranks, but I don't think that's very noteworthy given that all it really takes is consistently playing.
A rank that I could consider a real achievement was earning the codename FOX at the end of Metal Gear Solid 3: Snake Eater. At the end of each Metal Gear Solid game you get a codename which represents your overall performance during that playthrough. In order to get the codename FOX at the end of MGS3, you can't get spotted or kill any enemies, you can't use any healing items or continues, you have to save the game less than 25 times, and you have to finish in less than five hours.
It was super difficult but totally worth it since I'm a big fan of the series.
